There's either a variant for this piece or some instructions were printed wrongly.

This page http://www.peeron.com/scans/8855-1/9 (bottom right inset) shows the piece as it's currently modeled.
But this picture http://www.peeron.com/inv/parts/2712?img=22206 shows a notch in the axle hole.
Further, my printed instructions (printed in Germany, 1988), do show the notch.
