That's tricky - it's not really "open" if you can't fork it. If you can live with forkability I'd suggest either MIT/X11 or GPL - that's the two big flavors of open license and everyone understands them. When you go find some more specific and tweaky license, it might fit your desires 10% better than one of the big licenses, but it's also more likely that other contributors aren't going to understand the fine print.
